Best software to use for a simple questionnaire?

I need to create simple questionnaire to determine what software is required from various departments and what they don't need and what software doesn't work properly. With a nice heading etc. I'm not sure what to design this in. Maybe word . Not sure. Can anyone let me know what they use for surveys/questionnaires etc?
